Coordinated Care Center Management
• Develop Coordinated Care Centers (“CCCs”) as deemed appropriate.
• Determine legal compliance of CCC structure.
• Develop CCC pro forma.
• Identify and manage CCC space, leases, equipment and staff.
• Provide on-going CCC management.
• Educate providers as to the appropriate utilization of CCCs.
• Track and report weekly CCC performance.
• Develop marketing materials to promote the appropriate utilization of CCCs.
• Develop and manage, in conjunction with the Directors of Operations, patient recall, direct mail and other forms of CCC promotion.
